If you have any questions about our contract or liability waver please reach out to Catharine: 860-326-8148 catharine@luckydogct.com Lucky Dog Training Contract Lucky Dog operates as a positive reinforcement training facility. The use of corrective equipment (prong collars, choke collars, electric collars) are strictly prohibited within our facility. Additionally, physical corrections or verbal intimidation are not allowed in any of our training sessions. Should a client fail to adhere to this guideline, the Trainer reserves the right to ask the client to leave immediately, with no refund for services rendered. **Cancellation and Payment Policy:** If the Client fails to give at least 24 hours' notice to cancel an appointment, or is not present at the scheduled appointment time, the Client will be charged in full for the session. If payment is not made when due, the Trainer is entitled to suspend or cancel the Client’s program and require immediate payment of all outstanding balances. The Trainer will accommodate make-up classes for missed sessions within a 6-week period, subject to an additional fee. The Trainer agrees to provide the services specified below for the Client and the Client’s dog(s) (hereinafter referred to as “Dog”). The Client makes the following representations regarding their Dog: **Dog Health and Vaccinations:** The Client confirms that the Dog is up to date on all required vaccinations, including rabies, and is free from fleas, parasites, and infectious diseases. Proof of vaccinations must be provided before participating in training classes. **Services:** The Trainer will provide private lessons, classes, or hydrotherapy services for the Client and Dog based on schedule availability. The goal is to teach the Client how to train and work with their Dog. Training will occur at the Lucky Dog training location, the Client's home, or other locations as recommended by the Trainer. While the Trainer will make reasonable efforts to help the Client achieve their training and behavior modification goals, no guarantee is given regarding the Dog’s performance or behavior as a result of the services provided. The Client understands that they and all household members must follow the Trainer’s instructions without modification, work daily with the Dog as recommended, and continually reinforce the training. The Client agrees to maintain the Dog’s training post-program completion and acknowledges that training is a fluid process with no guarantees of specific results. As such, there will be no refunds for training. **Hydrotherapy & Conditioning:** The Client understands that the Trainer is not a rehabilitation veterinarian and is focused solely on conditioning and fitness goals. The Trainer will assess clients to determine their appropriateness for hydrotherapy and conditioning. If an underlying health condition is suspected, the Trainer reserves the right to decline service and refer the Client to a rehabilitation veterinarian. If the Client has an underlying condition, the Trainer may request a veterinary waiver confirming that the Client's Dog is physically capable of undergoing hydrotherapy. The Client acknowledges that they cannot hold the Trainer liable for any issues arising from hydrotherapy or conditioning sessions.

If you have any questions about our contract or liability waver please reach out to Catharine: 860-326-8148 catharine@luckydogct.com LIABILITY WAIVER In exchange for the opportunity to participate in the dog training offered by Lucky Dog LLC I state and agree as follows: I am aware and understand that there is always some unavoidable risk of injury involved when participating in activities with animals, including risk of injury to myself or my dog. I acknowledge the absolute responsibility to follow instructions, standards, guidelines and procedures established by Lucky Dog LLC its employees, or agents, and I take full responsibility for my actions and the actions of my dog. I am voluntarily participating in Lucky Dog LLC dog training activities with the knowledge of the risks involved and hereby acknowledge and agree to assume any and all risks of injury, death, and/or property damage, to myself or my dog, whether caused by the negligence of Catharine Garbo and Lucky Dog LLC its employees or agents, other participants, or otherwise. I hereby agree to forever release, acquit, discharge, and covenant to hold harmless Catharine Garbo and Lucky Dog LLC its employees, or agents, from any and all actions, causes of action, and claims on account of, or in any way growing out of, directly or indirectly, all known and unknown personal injuries or property damage which I may have or hereafter have whether attributable to the negligence of Catharine Garbo and Lucky Dog LLC, its employees, agents or otherwise. I agree to indemnify and hold harmless Catharine Garbo and Lucky Dog LLC, its employees and agents, for all costs and expenses which it may incur due to claims or demands alleging such an injury, and for any injury or damage caused by my dog to any other participant or dog, including settlement payments, court judgements, and legal defense fees. This Agreement is binding upon me, my spouse, and my children. This agreement supersedes all prior discussions, representations, warranties, and agreements of the parties, and expresses the entire agreement with Lucky Dog LLC regarding the matters described above. I confirm that, except for that which is specifically written in this Agreement, no promises, representations, or oral understandings have been made with regard to the behavior of any dog, the place where we are training, or anything else. This Agreement may be amended only by a written instrument signed by both me and Lucky Dog LLC BY SIGNING THIS AGREEMENT, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND ALL OF THE TERMS OF THIS AGREEMENT AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE COMPANY, EFFECTIVE AS OF THIS DATE.
